Asian Ginseng (Renshen): Textual History, Pharmacopoeial Identity, Chemistry, and the Limits of the Evidence

Asian ginseng is not the single word “tonic.” It is a materia-medica entity listed in the superior class of the Shennong Bencao Jing and staked by Zhang Zhongjing and generations of physicians on critical and depletion patterns. Its reputed actions rest on pattern-based combination and processing; its modern research (ginsenosides Rg1, Rb1) is largely preclinical, and human evidence for most outcomes remains inconclusive or conflicting.

1. Names and origins: the “man-root,” the “ghost cover,” and the “spirit herb”

The documentary trace of ginseng begins in the Shennong Bencao Jing, which ranks it in the superior class and describes it as sweet and slightly cooling, tonifying the five viscera, settling the spirit, calming palpitations, and brightening the eyes. Its very name — renshen, “man-root” — comes from the root's human-like form and its three-pronged, five-leaflet foliage. Later synonyms accumulate like sediment: “earth essence,” “spirit herb,” “blood ginseng,” “ghost cover,” “sea-fat” (haiyu). The poet Su Shi, who grew ginseng in his own small garden, called it haiyu in verse — a reminder that by the Song the herb had entered scholarly life as both crop and metaphor.

A caution for the modern reader: the classical renshen is not today's dangshen (codonopsis). The two are different plants in different families; codonopsis rose to prominence only after the Araliaceous ginseng of Shangdang was exhausted.

2. Source and authenticity: a thousand-year migration from Shangdang to the northeast

Ginseng's centre of production travelled steadily northeast. Warring-States and Han texts locate it at Shangdang (in modern Shanxi); by the Northern and Southern dynasties, Tao Hongjing's Mingyi Bielu first pairs Shangdang with Liaodong, admitting northeastern ginseng into the record. As Shangdang's wild stands were exhausted, the northeast became the sole authentic region, and “northeastern ginseng” came to dominate.

Statutorily, the Chinese Pharmacopoeia defines ginseng as the dried root and rhizome of Panax ginseng, usually dug in autumn; cultivated material is “garden ginseng,” while seed sown to grow wild under forest cover yields “understory mountain ginseng.” The pharmacopoeia assays ginsenosides Rg1, Re, and Rb1 as markers. Processing splits the drug in two: sun-dried “white” ginseng versus steamed “red” ginseng, in which heat converts part of the saponin profile toward a warmer character — the white-versus-red distinction is the starting point for understanding every ginseng product.

3. Actions and combination: from “greatly tonifying source-qi” to “dual tonification of qi and yin”

In Chinese medicine ginseng is sweet and slightly bitter, slightly warm, entering the spleen, lung, heart, and kidney, and heads the qi-tonifying category; its traditional actions are greatly tonifying source-qi, restoring the pulse and stemming collapse, tonifying spleen and lung, generating fluids, and calming the spirit. “Source-qi,” “spleen,” “heart” here are functional concepts, not modern physiological readouts.

Its combinations are strikingly graded. For collapse with a fading pulse, ginseng alone, strongly decocted, is Dushen Tang (Ginseng Alone Decoction). Add aconite for yang collapse and it becomes Shenfu Tang; add ophiopogon and schisandra for qi-and-yin damage with sweating and thirst and it becomes Shengmai San (Generate the Pulse Powder). For spleen-qi deficiency it joins atractylodes, poria, and licorice as Sijunzi Tang (Four Gentlemen), the parent of a whole tonifying family. One herb, three roles — restoring yang, astringing yin, fortifying the spleen — which is why its “effect” is never separable from its context.

4. How the physicians used it: Zhang Zhongjing's rescue and Li Dongyuan's tonification

Ginseng earned its throne at the bedsides of the dying. The Han physician Zhang Zhongjing loads ginseng into Sini-plus-Ginseng Decoction, Guizhi–Ginseng Decoction, Lizhong Wan (Regulate the Middle Pill), and Zhi Gancao Tang, using it for vomiting, diarrhea, and blood loss with a thready, failing pulse — establishing its role in stemming collapse.

In the Jin–Yuan era Li Dongyuan folded ginseng into his spleen–qi doctrine: Buzhong Yiqi Tang makes astragalus sovereign and ginseng minister to “tonify qi and raise yang,” while Sijunzi Tang makes ginseng itself sovereign, seeding later formulas such as Shen-Ling-Baizhu San, Bazhen Tang, and Shiquan Dabu Tang. The Ming master Zhang Jingyue paired ginseng with prepared rehmannia as the cornerstone of restoring the depleted. What the lineage teaches is precise: ginseng's “great tonification” was always a targeted tonification of a specific deficiency pattern, not a daily supplement for everyone — the historical mirror of the modern rule that a saponin, a sliced drug, and a finished formula are three different things.

5. Chemistry and pharmacology: reading ginsenosides Rg1 and Rb1

Modern phytochemistry sorts ginseng's actives into saponins (the ginsenosides — Rg1, Rb1, Re, Rd and others), polysaccharides, and volatiles. The ginsenosides, classed by aglycone into protopanaxadiol (e.g., Rb1) and protopanaxatriol (e.g., Rg1, Re) types, are the most studied and serve as pharmacopoeial markers.

Preclinical work is abundant and revealing of a complication: different ginsenosides often act in opposite directions (Rg1 tending stimulant, Rb1 tending sedative), so “ginseng” is chemically a committee, not a single agent. Cell and animal models suggest neuroprotective, immunomodulatory, anti-fatigue, metabolic, and cardiovascular activities — but these are mechanism, not human efficacy. NCCIH judges the diabetes blood-sugar literature inconsistent and conflicting; a review of 19 studies (~2,413 participants) found ginseng alone may modestly help general (non-disease) fatigue, while benefit in chronic fatigue depended on multi-ingredient formulas rather than ginseng alone, with small effect sizes.

6. From sliced root to product: red ginseng, extracts, injections, and international supplements

The product spectrum is wide: white, red, and understory-mountain sliced drugs; patent medicines such as Shengmai oral liquid, Shenfu preparations, Ren-Shen-Yang-Rong Wan, and Shen-Ling-Baizhu San; and TCM injections (Shenfu, Shengmai) with specific indications and monitoring requirements that demand medical supervision. Internationally, ginseng circulates mostly as standardized extract (labelled by Rg1+Rb1 content or as “red ginseng extract”).

Three boundaries matter. Red and white ginseng differ in saponin profile and emphasis, so their data are not interchangeable. A formula's or injection's results cannot be extrapolated to home-stewed ginseng or an ordinary supplement. And purifying a single ginsenoside (e.g., Rg1) as a drug candidate is a different technical path from sliced root in a decoction, falling under chemical-drug regulation once it becomes a medicine. The three evidence streams must not be conflated.

7. Dose, processing, and pharmacopoeial use (for professional reference)

The Chinese Pharmacopoeia gives a usual decoction dose of 3–9 g, decocted separately and added to the strained liquid; in acute collapse the amount may be increased under judgment and gently decocted apart. Choice of red versus white follows the treatment strategy — white for qi-and-yin deficiency, red for yang deficiency and interior cold. Classical incompatibility rules apply: ginseng should not be combined with veratrum and “fears” trogopterus dung. The dose range here is given to explain the standard, not as individualized advice.

8. Safety boundaries and interactions

NCCIH notes short-term oral use (up to ~6 months) at recommended amounts appears safe for most people, but long-term safety is uncertain. Insomnia is the commonest side effect; rare reports include severe rash, liver damage, and serious allergy. Ginseng may affect clotting and may lower blood sugar, and may worsen autoimmune disease; some experts advise against use in infants, children, and pregnancy/breastfeeding. People with diabetes, on anticoagulant/antiplatelet drugs, or on polypharmacy should consult a clinician first. Stop and seek care for palpitations, insomnia, blood-pressure swings, or rash, and disclose all ginseng products at clinical visits.

9. How to judge whether a claim about ginseng is credible

Frequently asked questions

With so much research on Rg1 and Rb1, doesn't that prove ginseng works?

No. Findings on neuroprotection, anti-fatigue, and cardiovascular effects are overwhelmingly in vitro/animal, and different ginsenosides often act in opposite directions. NCCIH judges the human evidence inconsistent or conflicting for most outcomes.

Can I swap red and white ginseng freely?

No. Steamed red ginseng has a different saponin profile and a warmer emphasis than white; the choice follows pattern differentiation and should be made by a professional.

The pharmacopoeia lists 3–9 g — can I just stew that at home?

Can ginseng affect blood sugar or blood thinners?
