Origin: the base type of the bupleurum formulas

Xiao Chaihu Decoction appears in Zhang Zhongjing's Shanghan Zabing Lun (c. 200 CE, later split into the Shanghan Lun and the Jinkui Yaolue) as the core formula of the shaoyang chapter and the base type of every later 'bupleurum formula.' The governing passage (Shanghan Lun §96) prescribes it for a patient five or six days into an illness with alternating chills and fever, chest-and-flank fullness, silent reluctance to eat, and irritability with retching, followed by a string of optional signs. More famous still is §101: when a bupleurum pattern is present, 'the appearance of a single sign is enough — they need not all be present.' This 'a single sign is enough' became the classical basis for the flexible use of the formula and for the wider diagnostic habit of 'seizing the chief sign.' The pair, one venting and one clearing, forms the skeleton of 'harmonizing shaoyang.' Because gallbladder heat invades the stomach and disrupts its downbearing, pinellia and fresh ginger harmonize the stomach and stop retching as assistants (ginger also restraining pinellia's toxicity); because the pathogen entered shaoyang on a background of deficiency, ginseng, jujube, and honey-fried licorice supplement qi and fortify the spleen, supporting the upright to expel the pathogen. The Qing physician Xu Lingtai's remark that 'the marvel of this formula lies in the ginseng' highlights how central supporting the upright is to 'harmonizing.' Cold and warm together, draining and tonifying together — hence its capacity to 'harmonize' disharmony between exterior and interior, organ and organ.

Pattern, not disease: 'half-exterior, half-interior' and the shaoyang pivot

Active constituents: saikosaponins, baicalin, and their double edge

Yet saikosaponins cut both ways. The literature notes a lack of systematic pharmacokinetic study, inconsistent doses across studies, contradictory experimental evidence, and a risk of hepatotoxicity on overdose — a modern footnote to 'benefit and toxicity share one source.' Evidence, and a safety history that must be told: the TJ-9 interstitial-pneumonia episode

As for efficacy, most work is in-vitro, animal, and preliminary clinical observation, lacking large high-quality RCTs and often mixing preparations and co-therapy. The defensible statement is that mechanistic and preliminary clinical research exists alongside clear preparation-related safety signals — not that the formula is proven to treat hepatitis and is safe.

Quality standards, patent medicines, and the Western-drug context
