The four-layer institutional map

Item listing defines which service can stand as a price item; price setting determines the local price or method; payer policy determines benefit scope and settlement; oversight tests whether care was delivered, billing matched the item, and funds were used properly. A national listing guide informs local implementation but does not itself create a local charge.

Effects under three payment contexts

Under fee-for-service, item scope, unit, and included/excluded content govern separate billing. Under DRG/DIP, many services sit inside a case payment, yet patient charges, payer settlement, and provider cost remain distinct. Per-diem and other fixed payments require controls against under-service and improper self-pay shifting.

• Clinician: document medical necessity and actual delivery.
• Patient: distinguish posted price, benefit status, and personal liability.

Oversight and internal control

Common risks include charging without authority, duplicate or fragmented billing, over-standard charging, substitution, and fictitious service. Systems should validate context, frequency, unit, exclusions, and evidence. An anomaly starts review; it is not a standalone legal conclusion.
